Phase 2: Transition to Independent Practice (2–6 Weeks)
Focus: Clinical prioritisation, communication, and foundational cardiology skills
Learning Objectives
- Prioritise and organise care for a group of patients under supervision
- Accurately interpret and respond to Q-ADDS scores with appropriate escalation
- Develop foundational cardiac condition knowledge including ACS, heart failure, electrophysiology, and valvular disease
- Deliver patient education using clear communication tailored to the individual
- Identify opportunities to contribute to discharge planning alongside team members
Learning Activities
- Enrol in TPCH cardiac monitoring modules (discuss with nurse educator)
- Prioritise and organise care for a group of patients under supervision
